Dearest grandpa of Kyle Jendrzejak, Kevin Jendrzejak and Sarah Polivka. Cherished son of the late Helen and the late Richard Polivka. Dear brother of the late Richard Jr. (Donna) Polivka. Also survived by many nieces, nephews, relatives and friends.
